  Office Hours: 8:00-5:30 Monday thru Thursday 8:00-5:00 Friday
  • We are closed from 12:00 to 1:30 for lunch.
  • Please bring all MRI, X-RAY, CT FILMS AND REPORTS with you if you have had these procedure done prior to your appointment with us.
  • If you must reschedule or cancel your appointment, please call us so that we can give your time to someone else. Repeated cancellations, no-shows, and reschedules will adversely effect your ability to schedule with us in the future.
  • All pre-op’s are scheduled at least one week in advance of your surgery. This is necessary in order to complete labs, if needed, and to take care of any other outstanding medical issues that may arise. You will also be given information  when you pre-register at the hospital. The hospital will give you instructions on when to arrive, what to bring, when to stop eating/drinking, and any other  information specifically regarding your surgery.
  • You are responsible to obtain a referral from your primary care provider (PCP) if required according to your policy. Some HMO’s require this. Call your family practice physician for information.
  • Please arrive at least 15 minutes prior to your scheduled appointment time on your first visit to complete all paperwork. This will help us stay “on time” with all our patients.
  • All prescription refills are handled on Tuesdays and Thursdays. Call your pharmacy and ask them to fax us a refill order. Once it is authorized by the doctor, we will call it in to have it filled. DO NOT try to fill the same prescription at more than one pharmacy.
  • In an effort to protect your medical and health information, this office follows the HIPPA regulations. We are happy to provide you with a  copy of these regs and a copy is posted in our waiting room for your convenience.
It is our intention to provide you with the best possible care. These policies are in place to help achieve this and provide you with a good experience.
